The U.S. Department of Defense has introduced comprehensive guidelines for a newly mandated testosterone deficiency screening program targeting active-duty and reserve service members aged 30 and above. This initiative follows a recent announcement made by U.S. Secretary of Defense Pete Hegseth in July.

Effective immediately, the guidance outlines standardized procedures for screening and treating testosterone deficiencies in both male and female service members. The Pentagon asserts that these measures aim to enhance military readiness by identifying and addressing hormonal and energy-related issues.

Hegseth, a proponent of testosterone use, unveiled the mandatory screening policy, sparking discussions among experts regarding its scientific validity. The U.S. Food and Drug Administration is scheduled to convene a panel of experts in mid-September to deliberate on the medical application of testosterone.

Under the new guidelines, men aged 30 and above will undergo compulsory testosterone blood tests, while younger men will be tested upon request or if symptoms are observed by healthcare providers. For women, routine testosterone tests are not mandated, but screening for fatigue and menstrual irregularities associated with hormonal imbalances and energy deficiencies is recommended.

The guidance also provides directives on prescribing off-label testosterone for women, particularly postmenopausal individuals experiencing significantly reduced sexual desire. Some medical professionals have expressed concerns that widespread testosterone testing may result in unnecessary or potentially harmful treatments, emphasizing the lack of substantial evidence supporting universal screening for low testosterone in military personnel to enhance combat readiness.
